Parental Permission to Publish

St. Margaret's Day School may publish photographs of our students in various forms of media. We are asking for your consent to use your child's photograph in publications that promote our school. Some of these publications and materials may continue to be used after your child is no longer enrolled at St. Margaret's.
If you desire to provide consent, please indicate below. You may revoke your consent at any time by providing written notice to the school. If you do not wish for your child to appear in any publicity regarding the school, we will certainly respect your request.
No names will ever be published in conjunction with your child's photo.
