Welcome to a new year at SMDS! Please make sure you have completed all of the below forms that apply to your child.

Health and Emergency Form 

​Health Inventory Form​ (print and return, part II must be completed and signed by a physician)

Allergy Form (only needs to be completed if you child has a known allergy) 

​Medication Administration Authorization (print and return if applicable)

Lead Testing Forms (all children born after January 1, 2015 must have the lead testing completed)

If your child is entering a 4s, Pre-K, or Kindergarten class, please print and return the Early Screening Inventory Parent Questionnaire linked below. 

ESI-R Form

Below are the Ages and Stages Questionnaires for each age group in the 2's and 3's. Please print and fill out the form relevant to your child and return to the school.
